Returning athletes entering 8th through 12 grade:
After 6/1/22, you and your son/daughter will need to log into your PlanetHS accounts and update the forms: Click here to log in. You do not need to create a new account for yourself or your son/daughter. If you have any questions or issues, please contact customer support at 866-448-9438 or [email protected]. Once all the forms are updated your student-athlete will need to get a new 2022-23 athletic physical. Physicals for the 2022-23 school year must be done on or after 6/1/22. Any physical done prior to this date is acceptable as long as the medical professional who performed the exam, reviews the physical and then signs and dates the new physical form with the original physical date and the date in which he/she reviewed it.
Athletes entering 7th grade or new student-athletes to QVSD entering 8th through 12th grade
Starting last year, QV athletics went paperless for most, if not all paperwork (physicals, medical release forms, zero tolerance forms, etc) required to participate in athletics at QV. Each parent/guardian and student-athlete must create an account here. Once both the student-athlete and their parent/guardian have created an account, you will link the accounts together and all the required documents will be there to complete. All signatures will be done electronically. The PIAA physical packet will be filled out in the student-athletes account and the actual physical form that the doctor/approved medical provider fills out will then be uploaded by you. The site is extremely user-friendly and will walk you through the entire process. We will not be collecting any hard copies of these forms so you must create an account to complete all the required paperwork. Free athletic physicals are being offered by Greater Pittsburgh Orthopedics and Greater Pittsburgh Physical Therapy on Monday, June 13th. These physicals are only for student-athletes entering 9th through 12th grade. For information on how to sign up for a physical please click here.
Regardless of where your son/daughter gets their physical completed, you will need to print and fill out Section 6 of the PIAA Physical Packet (Medical History Form) and Section 7 (PIAA Pre-Participation Physical Exam Form) and take those with you to the physical appointment Once you have a signed and dated Section 7 completed for your son/daughter, that form needs to be uploaded to their PlanetHS account. Remember the form must be signed and dated by the appropriate medical professional on or after 6/1/22. In order for your son/daughter to be eligible to participate in a sport at QV, all their forms must be completed and electronically signed by the parent/guardian and the student-athlete. Failure to have all forms and signatures completed will result in your son/daughter not being cleared to participate.
Baseline ImPACT Testing:
Baseline ImPACT testing will be done at home. Please click here for information on who needs to take a new/updated baseline ImPACT test and how to take the test at home. Testing can be done starting Monday, May 30th.
